There can be practically any reasons for changing the last name . The most common, of course, is marriage. According to statistics, 80% of brides at the time of marriage prefer to take the name of the spouse.
Some want to change their own not too harmonious surname (for example, Svinkin, Rylov, Fools, etc.). The law does not determine the range of reasons for which a change of surname is possible. Any citizen of the Russian Federation who has reached the age of 18 is entitled to change his name on his own initiative, and has reached the age of 14 with the consent of both parents (trustees).
Change of surname, documents for exchange
First of all, it is necessary to determine which documents to change when changing the name. Substitutions require almost all documents in which your last name is indicated, but it is better to separate the main list and the list of less important documents. The main ones include: passport, medical insurance policy, SNILS, TIN. A minor list includes a passport, credit cards, driver’s license, etc.
Passport
The passport is the main document, it is changed first of all, when the name changes. Documents to be submitted to the passport office:
- application for a change of last name;
- Original passport
- receipt of payment of state duty;
- photo (4pcs.).
Pay the state fee in advance, as well as take photos. Sometimes photos can be taken right in the passport office, but if you do not know for sure this is better to prepare. Otherwise, everything is very simple, you need to fill out an application and submit all the documents. You are required to replace your passport in 10 days, sometimes the time period may increase up to 30 days, but no more.
SNILS
Next, it is necessary to replace the SNILS, as it may be required when applying for a medical policy. There will be no particular difficulties in exchanging a pension certificate . If you work, then you just need to hand over the old SNILS to the personnel department for an exchange. The employer organization is responsible for exchanging your certificate on its own. If you do not work, then contact the Russian Pension Fund, fill out an application for exchange of certificate, have a new passport (and its photocopy) and your old SNILS and marriage certificate (and its photocopy) with you.
Compulsory Health Insurance Policy
Be sure to replace the health insurance policy if there is a change of surname. Documents to be submitted to your insurance company: passport with a new name, old policy, marriage certificate. You are required to issue a temporary policy and inform you when a new one will be ready.
TIN
TIN is another important document to be exchanged if a name change is required. Documents that should be submitted to the tax office: passport with a new name, old TIN and a copy of the marriage certificate (the original should be with you). It remains only to fill out an application on the model and wait one week.
You can determine what documents are needed to change the surname, in addition to the main ones, depending on your plans. For example, a driver’s license cannot be exchanged, but then you must always carry a marriage certificate in the car. An international passport is subject to exchange only if you plan to travel abroad while your old passport is considered valid. Credit cards are subject to exchange, if they are registered and you constantly use them, conduct transactions through the bank.
