A personal medical book is needed for representatives of the professions involved in the production, storage, transportation and sale of food and water, training and parenting, utilities and consumer services.
The list of activities is established by order of Rospotrebnadzor No. 402. According to the legislative act, a personal medical book should be employees who are involved in the following areas:
- the turnover of food and food raw materials in the food industry, trade, catering;
- educational and recreational activities for children and adolescents in kindergartens, schools, colleges;
- household organizations such as hairdressers, hotels;
- trade in manufactured goods (clothing, toys, household appliances, spare parts for cars);
- drivers of urban passenger transport (bus and trolleybus depots, metro, shuttle buses);
- medical workers.
A personal medical book is kept by the organization’s administration or individual entrepreneur, issued to the employee:
- on demand;
- upon dismissal from work.
The medical book is extended according to the schedule established for each specialty. The document must be transferred to the next place of work.
Registration and registration
Since 2001, the Federal State Health Institution “Center for Hygiene and Epidemiology in the City of Moscow” has organized an electronic Register of issued personal medical books, which records the results of a professional hygiene check.
According to the document, a personal medical book was equated with strict reporting documentation. After passing the mandatory preventive medical examination, the employee is obliged to enter the results in the medical book. The document reflects hygienic certification.
Who issues medical books?
Only organizations authorized by Rospotrebnadzor can issue a document - hygiene and epidemiology centers. The official status of the personal medical book is confirmed by:
- holography;
- the seal of the organization of Rospotrebnadzor issuing the document;
- approval of the certification result by a stamp of an organization authorized by Rospotrebnadzor in order No. 402.
This medical book in Moscow is not available for free purchase and sale. When undergoing hygienic training, the document is handed out, must be registered.
Order of registration
To issue a medical book, an employee must present:
- a written application for extradition;
- passport;
- photo (3 by 4 cm);
- referral from the employer with the name of the place of work;
- registration certificate in Moscow or the Moscow region (this applies only to nonresident).
Documents can be submitted personally by the applicant or a proxy with the power of attorney.
The completed personal medical booklet obliges the applicant:
- Pass examinations in licensed medical facilities.
- To study under the program of professional hygienic preparation.
Persons who have passed hygienic training and received a doctor’s opinion on the result of a medical examination are allowed to certification. The results are recorded in the personal medical book in accordance with the procedure approved by order. That is why you need to be interested in the availability of a license from the center issuing the document, as well as the availability of hygiene training programs.
Deadlines
Making a medical book fast means spending at least 5-7 days. One-day and even half-hour filling is equivalent to a fake document. The procedure is established by law. Any acceleration of the process will amount to a violation of the law.
Obtaining and prolonging medical books involves several stages in a specialized clinic or center:
- Fill out the bypass sheet, which is issued at the registry.
- Pay for a medical examination.
- Take tests, do fluorography, get around the doctors.
- Contact the registry with a completed bypass sheet, get a tear-off ticket for passing a medical examination. It indicates the time of receipt of the medical book.
- Appear for hygienic certification and pick up the document.

Specialized Centers
Medical centers that are accredited by Rospotrebnadzor and the Moscow Center for Epidemiology provide a full range of services in accordance with the law. By order No. 302n, these medical institutions conduct medical examinations and draw up books with entries in the state register.
Officially, a medical book in Moscow is not issued within one day just because receiving the results of a blood test requires more time. Employees of medical centers try to make the examination as easy as possible, arranging the stages of passing specialists and passing tests so that a busy person does not spend more than 40 minutes. The real period for receiving a medical book officially (by all the rules) is 5-7 days.
List of compulsory examinations
The list of doctors who need to be circumvented and analyzes depends on the specialization of the employee:
- The standard set of examinations for most specialties includes: fluorography, a blood test for syphilis, gonorrhea and Trichomonas. Need a detour from a therapist and a dermatovenerologist.
- For professions in contact with children, patients and food products, dental and otolaryngological examinations will be required. A throat smear for Staphylococcus aureus, a study for helminths, enterobiasis and typhoid fever is required.
- An employee with harmful conditions will need to have a biochemical blood test to determine the levels of bilirubin, enzymes and liver proteins.
- Health workers need to donate blood for viral hepatitis C and HIV.
A timely medical examination for a medical book reveals dangerous diseases, prevents infection of children, weakened persons and the spread of infections. If microorganisms or antibodies to them are detected, the employee is sent for additional examination, not allowing them to work.
What to do if the expiration date of the book
The prolongation of medical books is a confirmation of the employee’s health by taking tests and passing a medical examination again on time. Appendix No. 2 to the decision of the Moscow government governs two aspects of the procedure:
- renewal frequency;
- the frequency of analyzes and examinations is determined depending on the specifics of the work.
Workers in kindergartens, schools and other professionals working with children need to undergo a medical examination on a quarterly basis. Industrial workers - once a year. A personal medical book serves as a document in which data is entered from year to year. After entering the next data, the authorized person must:
- make a note on the extension of the validity of the book;
- enter data in the state register.
Without the above actions, the payment for the medical book does not make sense, since only registration makes the extension of medical books legal.
Why do you need to pay for registration?
Free to issue a personal medical book in the clinic will not work. You do not need to pay for fluorography and vaccination on schedule. The cost of a medical book, in addition to the costs of each medical center, includes:
- lecture or hygiene training expenses;
- medical booklet form;
- tutorial.
The data of the official website of the Federal Service for Supervision of Consumer Rights Protection and Human Welfare determine how much the medical book costs at its initial design. This complex cost an average of 800 rubles at the beginning of 2017. A referral to a dermatovenerologist, blood tests are given in clinics at the place of residence. You will need to adapt to the specialist’s work schedule, queue up, pay in the end for an individual approach to speed up the process. Extending a medical book in a specialized center provides the main advantage - it saves time.
Examinations and analyzes at the clinic often turn out to be paid and unavailable due to queues or lack of reagents.
There is a concept of expiration date of the main analyzes:
- blood, urine, infections and throat swabs - up to 10 days;
- analysis for HIV and hepatitis - 90 days;
- bacteriological culture and cytology - up to 20 days;
- helminth analysis - 30 days.
Medical examinations are valid for a month, as are the results of ultrasound of the abdominal organs. Mammography should be done every two years.
