3. The essence of the appeal is recorded sequentially:

  • the request itself (“please provide”);
  • object of the request (annual vacation);
  • start and end dates (“from __” and “to __”);
  • number of days ("for ___ calendar days");
  • basis (vacation schedule).

But it happens that the reason does not fit into this framework. Then the head of the enterprise must independently decide whether the reason for the absence of the employee at work is valid or not. Such leave is granted only by agreement of the parties and becomes effective after the issuance of the relevant order.
