High-tech medical care, in other words, VMP is a complex of diagnostic and therapeutic medical services carried out in a hospital using unique and complex medical technologies, which have the resource-intensiveness of medical technologies and have a finished clinical value. Quotas for high-tech medical care are allocated and funded by the federal budget. The list of methods, types and forms of medical care that are high-tech is approved each year by the Ministry of Health and Social Development of Russia in accordance with the needs for VMP for each individual region. In other words, the regions determine the quotas for high-tech medical care on their own, taking into account the needs of residents in VMP based on the dynamics and structure of the incidence in their region.
The quota for treatment that is issued to the patient covers all the costs of treatment that is carried out in a specialized medical center, including staying in the hospital in the postoperative period and the hospitalization period, as well as the cost of medicines.
Quotas of the Ministry of Health, which are issued for the provision of high-tech medical care, are entitled to receive all citizens of the Russian Federation without exception. The main criteria for getting help are medical indications.
Currently, the allocation of quotas for high-tech medical care is carried out by three sources of financing.
- Treatment of patients and provision of medical care in federal medical specialized institutions at the expense of federal quotas. The direction of patients according to federal quotas is determined by the Order of the Ministry of Health and Social Development of the Russian Federation. The choice of federal medical institutions depends on many factors, including the availability of a transport scheme and long-term cooperation with various research centers. Compared to previous periods, federal quotas are now most actively issued in cities such as Noyabrsk, Novy Urengoy, Nadym and regional cities, Salekhard.
- Referral of patients to state medical institutions and institutions of other subjects of the Russian Federation over territorial quotas.
- Referral of patients for treatment due to regional targeted programs.
How is assisting at the VMP?
The basis for the decision on the allocation of quotas for high-tech medical care are the conclusions and recommendations of the chief expert expert on the need for the provision of VMP, which are justified:
1) medical documentation of the patient;
2) the lack of the ability to provide the necessary assistance to the patient as part of the territorial program.
On the day of sending the patient's documentation (test results, extract from the medical history, laboratory, clinical, radiological studies that relate to the disease, not more than a month ago) for consultation at the FSMU. The commission fills out a medical assistance coupon, which is sent along with the documentation to the FSMU. The results of the decision taken by the FSMU are sent back to the Commission of the subject of the Russian Federation. In the event of a positive decision to provide the patient with the necessary medical care, the Commission shall notify the medical institution that sent the documentation of the date of hospitalization, issue coupons to the patient and his companion to receive preferential travel documents, fill out and issue with the stamps and signatures a VMP referral coupon. In other words, it issues a quota for high-tech medical care free of charge.
