In 2013, instructions were approved for filling out a new form developed by the federal statistical authorities on the provision of information regarding injuries due to industrial accidents.
Form 7-injuries are given by organizations and enterprises once a year. Once every 3 years, the TOGS provides an Appendix to the report of the 7-injury form, which reflects information on the distribution of victims of industrial accidents due to accident reasons and their main types.
Who should take the 7-injury form?
These statistics should be submitted to statistics by organizations of all forms of ownership, with the exception of:
- microenterprises;
- legal entities whose activities are related to public administration and ensuring military security;
- organizations whose activities are related to finance, social insurance and the provision of insurance services.
This list also includes households and extraterritorial organizations.
It is necessary to submit the 7-injury form to the territorial office of Rosstat before January 25.
Information on microenterprises and extraterritorial organizations
Micro-enterprises include companies and organizations with less than 15 employees. This definition is enshrined in the Law on Small and Medium Enterprises on the territory of the Russian state.
Extraterritorial organizations are representatives of other countries located on the territory of Russia, and international companies.
The recommendations on filling out the form established by the federal statistical authorities stipulate that the above organizations, regardless of their form of ownership, do not submit reports to injury statistics.
What do injuries indicate in form No. 7?
The report contains data on injured workers as a result of cases that caused injuries in the workplace. Both the main document and the Appendix to the form 7-injuries are filled out on the basis of an act drawn up as a result of the investigation and filled out in accordance with the corresponding form (H-1). The procedure for the management of an enterprise, organization or institution during a traumatic situation is enshrined in the Labor Code of the Russian Federation (Articles 227 to 231).
The application form for the form 7 injuries can be downloaded in the legal information systems “Consultant” or “Guarantor”, as well as on various Internet sites.
Forms required to fill out documents at the time of an accident during the production process are approved by law.
Basic rules for filling out the Annex to the form 7-injury
- All information specified in the Appendix to the main report should reflect the completeness of the data and their reliability.
- All data entered in the table are indicated in units, which are assumed by the report form.
- If the legal entity has in its composition divisions operating separately, then the Appendix to the form 7-injuries shall be filled out separately for each.
- Ready-made reports are submitted to the bodies of Rosstat at the place of registration of each separate division and the legal entity itself. If the organization does not operate at the place of registration, then the data is provided at the place of actual location.
- In the line where it is required to indicate information about the enterprise, write the full name of the organization, according to the constituent documents, and in abbreviations indicate the abbreviated name, if any. If the Appendix to the form 7-injuries is filled in on a separate unit, then first indicate its name, and then the name of the main organization to which it relates.
- In the line where you want to specify the address, registration data and the actual location are prescribed. Index is also required.
- The All-Russian classifier of organizations and enterprises, which is indicated by legal entities in the code part of the application, is assigned and sent to them by the territorial statistical authorities.
The head of the organization is the responsible person who will be involved in reporting and filling out the statistics form. The appendix to form 7 injuries are usually filled by accounting personnel or labor protection specialists at the enterprise.
Important details when filling out the Application
All data on the victims of accidents at the enterprise, which are entered in the Appendix to the form 7-injury, must comply not only with the act drawn up as a result of the investigation of the event that caused the injury, but also with all the requirements and rules enshrined in the legislation of the Russian Federation.
When filling out the Appendix to the form 7-injuries, it should be ensured that the following values coincide with the main report:

How to fill out an Application to the form 7-injuries
The number of victims for the main types of adverse events that led to a sad outcome (these are lines 2 to 15 in the Appendix) are filled out on the basis of the entry in the Act, compiled from the investigation of what happened at the factory (take into account the item in the act called “Type of incident” )
Separation of victims for the reasons of what happened (these are the remaining lines from 16 to 30) are filled out on the basis of the same act, they only use the item called "Causes of the accident".
Line 01 indicates the sum of lines 2 to 15 and 16 to 30. Accordingly, the values obtained should be equal, since only one number is written. In the 3rd column indicate all the victims of the accident, and in the 4th column indicate the number of those whose accident was fatal.
Arithmetic and logical calculation
- The first line of the third column corresponds to the sum of lines 2 to 15 and the sum of lines 16 to 30.
- Row one of columns four equals the sum of rows 2 through 15 and the sum of rows 16 through 30.

As already mentioned, the form No. 7-injuries and the Appendix to it, which contains information on the breakdown of injured people by type of cases and reasons, are filled out for each unit of the legal entity that operates separately, separately. It should be remembered that they include even those units that are not fixed in the constituent documents, but are recognized as such.
Responsibility for failure to report
If the organization neglects the rules for providing information on the number of accidents and the number of injured people, then administrative responsibility awaits it.
If the information provided was distorted (erroneously or intentionally), then the company indemnifies the damage to the statistical authorities, as they will have to amend the already prepared reports in the future.
