A symptom of Ortner-Grekov is manifested in diseases that are associated with the liver or
biliary tract. Most often it is observed in acute cholecystitis. The symptom is characterized by a certain soreness on the right side with lightly striking with the edge of the palm of the palm on the right along the costal arch. It is mandatory that striking is carried out on both sides in order to compare and determine the Ortner-Grekov symptom with accuracy. The pain occurs due to the fact that the doctor produces a slight concussion of the
inflamed gallbladder. Under normal conditions, patients may sometimes not feel pain, feeling only a certain heaviness on the right side under the ribs.
Acute cholecystitis
Acute cholecystitis, according to statistics, is diagnosed in 18% of patients who have acute surgical diseases of the abdominal cavity. In women, the probability of its occurrence is three times higher. The causes of this disease are many, as are the chances to fully experience the Ortner-Grekov symptom. These include:
- hypertension in the biliary tract;
- infection in them;
- changes in the vessels of the biliary tract;
- violation of the diet;
- cholelithiasis;
- decrease in nonspecific resistance of the body;
- diseases of the stomach, which may be accompanied by dyscholy;
- morphological problems (for example, with the liver - the development of hyperbilirubenemia).
Almost 90% of cases of exacerbation of the disease are most often the result of complications of gallstone disease. 100% nutritional factor (diets) are the trigger. Fatty and spicy foods, allergens can lead to such a disease. All this develops a spasm, which determines the symptom of Ortner-Grekov. But the diagnosis should be carried out only by a doctor.
Diseases that provoke cholecystitis
We have already examined the problems with cholelithiasis and diet. It is worth talking about some other diseases that lead to problems with the gallbladder. So, for example, gastritis, which may be accompanied by a decrease in the production of gastric juice and, in particular, hydrochloric acid, also sometimes leads to the fact that during diagnostic measures a positive symptom of Grekov-Ortner is revealed, which sometimes leads to an incorrect diagnosis.
Definition of the disease and its degree
The stage of development of the disease depends more on what changes have occurred in the gallbladder, how the process is running, and whether complications have manifested. In the case of the latter, not only a positive symptom of Ortner-Grekov is characteristic, but also there is an irradiation of pain in the shoulder, scapula, supraclavicular region.
Other symptoms
Among the concomitant symptoms of acute cholecystitis are nausea, vomiting with bile, bitterness in the mouth, dull pain at the initial stage and its progression with the development of the disease. Body temperature can rise to 38 degrees with the presence of chills. There is an increase in heart rate, in some cases, tachycardia occurs. Jaundice may also occur. The tongue becomes dry and overlaid. All treatment should be prescribed by a doctor. Running cholecystitis can cause concomitant diseases with subsequent failure of the affected organs.
