Corinfar is a calcium antagonist, that is, it inhibits the penetration of Ca2 + ions into the muscle cells (cardiomyocytes) of the heart and blood vessels from the intercellular space. Thus, there is a decrease in the concentration of calcium ions. As a result, peripheral, coronary arteries and arterioles (soft arteries) expand. Pills, having a prolonged effect, have a pronounced vasodilating effect.
The medicine "Corinfar" is available in the form of round biconvex tablets, coated with a yellow film membrane. One tablet contains the active substance - nifedipine (synthesized artificially from dihydropyridine) - 10 mg. As excipients that fill the tablet, lactose monohydrate, povidone K25, potato starch, microcrystalline cellulose, magnesium stearate are used. Corinfar is sold in cardboard packaging with three blisters (10 tablets each) or in dark glass bottles of 50 and 100 tablets.
In diseases such as angina pectoris, arterial hypertension (persistent high blood pressure), patients are prescribed Corinfar. Instructions for use are in Russian. All information is useful not only for medical workers and doctors, but also for patients, as it will help to treat the prescriptions of the doctor with understanding, as well as to changes in the body that are the result of exposure to this drug.
It is known that the therapeutic effect will be delayed (without reducing the absorption of the active substance - nifedipine) if Corinfar is taken with food. Instructions for use recommend this (without chewing) after eating and drink plenty of fluids. Duration and dose (i.e. course of treatment) are selected individually and are prescribed by a doctor. Sensitivity to corinfar and the severity of the disease must be taken into account. For elderly patients and those suffering from concomitant severe cerebrovascular disease, the dose is reduced.
Start with 10 mg of nifedipine (or 1 tablet two to three times a day) to take Corinfar. Instructions for use allow a gradual increase in dose to 2 tablets or 20 mg once or twice a day. The daily maximum dose should not be more than 4 tablets or 40 mg. With essential hypertension (hypertension), the recommended average daily dose should be 1 tablet or 10 mg (considered nifedipine) two to three times a day. To achieve the necessary clinical effect, the dose prescribed by a doctor can be increased to 40 mg (for nifedipine) per day, which should be taken 2 times. Between two doses of Corinfar, with two or three times the appointment, the time interval should be at least four hours.
In some diseases, it is not recommended to prescribe Corinfar. Instructions for use indicate its contraindications for hypotension, cardiogenic shock, collapse, chronic heart failure, with severe aortic stenosis, unstable angina pectoris, acute myocardial infarction (during the first four weeks), combined with rifampicin. There are contraindications also during lactation, in the first trimester of pregnancy, with increased sensitivity to nifedipine or to auxiliary components of the drug.
Particular care is required for hypertrophic obstructive cardiomyopathy, mitral valve stenosis, sinus node weakness syndrome, severe tachycardia or bradycardia, hypovolemia, malignant arterial hypertension, myocardial infarction with left ventricular failure, severe cerebrovascular accident. Also, with renal and hepatic insufficiency, gastrointestinal obstruction, hemodialysis, during the periods of II and III trimesters of pregnancy, under the age of 18 years, with the simultaneous use of beta-blockers, digoxin.
A drug that has a pronounced antihypertensive and antianginal effect with a prolonged effect due to the delayed action of the active substance is called Corinfar Retard. The manual contains information for healthcare providers and patients. The active substance of the drug is easily absorbed in the digestive tract. The therapeutic effect develops no more than 15 minutes after taking the medicine. The metabolism of Corinfar components occurs in the liver due to hydrolysis and oxidation. With the metabolism of nifedipine, inactive compounds are formed. Corinfar is excreted mainly by the kidneys, as well as with feces.
