Arterial hypertension is a disease of millions of people nowadays. The drug "Valz" refers to peripheral vasodilators, that is, expanding blood vessels, leads to a decrease in blood pressure. This effect is due to the active substance - valsartan, which is a specific blocker of AT1 angiotensin II receptors, does not inhibit ACE, does not affect the amount of total blood cholesterol, and glucose, uric acid, and triglycerides.
The primary effect is manifested 2 hours after the use of the drug, achieving maximum effect after 4-6 hours. The therapeutic effect can last more than 24 hours. Regular intake of tablets leads to the most persistent decrease in blood pressure after 2-4 weeks. If there is a need for a sharp cessation of taking the drug, withdrawal syndrome is not observed.
Mechanism of action
After oral administration, rapid absorption of the active substance occurs against a background of varying degrees of absorption. The absolute bioavailability index is on average 23%. After oral administration, it binds to plasma proteins by almost 97%. Being an antagonist of angiotensin II, the active substance causes the expansion of the walls of the blood vessels of the bloodstream, which leads to a decrease in blood pressure in it. The drug is processed using a specific enzyme system for 9 hours, after which it is excreted 70% through the intestinal tube and 30% using the urinary system in a slightly modified form.
Taking a single daily dose leads to a very small accumulation of the active substance in the body.
Indications for use
The drug "Valz N", analogues of which are prescribed for similar pathologies, use:
• with high blood pressure;
• in chronic heart failure as a component of complex treatment;
• after acute myocardial infarction (prescribed from 12 hours to 10 days), complicated by the occurrence of left ventricular failure, systolic dysfunction of the left ventricle against a stable level of hemodynamics.
Contraindications
Analogs of the drug "Valz", like himself, are not used:
- with hypersensitivity to the active component or excipients;
- with violations in the liver (especially with the development of cholestasis or biliary cirrhosis);
- with deviations of various kinds in the functioning of the kidneys;
- at different stages of pregnancy and during breastfeeding;
- in children under 18 years of age;
- with galactosemia due to malabsorption of galactose and with lactose intolerance.
Low blood pressure, insufficient potassium and sodium in the body, bilateral renal artery stenosis, aortic and mitral stenosis, cardiomyopathy, and primary hyperaldosteronism require careful use of the Valz drug. Side effect

Dosage and dosage regimen

To reduce blood pressure, tablets are prescribed in a dose of 80 mg (single and daily at the same time). With the development of chronic heart failure (CHF), therapy is started with a dose of 40 mg, prescribing it 2 times a day. If the desired effect is not observed, the patient is transferred to "Valz 80" - an analogue of the drug "Valz 40", also with a double daily intake (maximum dose - up to 320 mg in two doses). It can be combined with other means designed to overcome chronic heart failure.
Against the background of a myocardial infarction in the anamnesis with a stable level of blood flow, valsartan therapy can be started within 12 hours after the relief of an acute condition. At the same time, half of the Valz 40 tablet is prescribed in the morning and evening, gradually increasing (over several weeks) to a dose of 160 mg twice a day, passing through dosages of 40 and 80 mg, respectively. For patients with mild to moderate hepatic impairment (without cholestasis, in the presence of which the drug “Valz”, Russian analogues, as well as foreign ones, are not prescribed), the permissible daily dose of this drug is used up to 80 mg of the active component.
Application features
Since this substance has enough contraindications and side effects, it should be used with caution, observing all the doctor's recommendations and strictly adhering to the prescribed dosage, especially if there are deviations in the work of other organs and body systems.
If the patient has a pronounced sodium deficiency, combined with a decrease in BCC (which can occur due to the intake of a large number of diuretics), treatment with Valz N (an analog of Valz) can lead to a pronounced drop in blood pressure at the very beginning of therapy , which will negatively affect the well-being of the patient. To avoid such an effect in the presence of a similar problem, it is necessary to restore the level of electrolytes and their ratio in the body before using these products.

The use of the drug “Valz 160”, analogues of which are not prescribed for this disease, for the treatment of patients with heart failure, can cause a drop in blood pressure, which requires monitoring of its performance.
For patients suffering from unilateral or bilateral renal artery stenosis, it is necessary to establish a regular measurement of serum urea nitrogen and creatinine levels.

Use during pregnancy and lactation
The use of Valz tablets while carrying or breastfeeding is absolutely contraindicated. This is due to the fact that studies on the effect of valsartan on the fetus were not conducted.
