Adenomyosis or otherwise endometriosis is a disease of the uterus. In the course of this disease, the endometrium, which acts as an internal mucosa, begins to grow into various layers of the uterus. General description of the pathology

Pathology is divided into diffuse, nodular and mixed form.

As already noted, adenomyosis is a benign pathological growth that occurs in the tissues. Another symptom typical for the described disease is pain, which appears before menstruation and lasts for several days. Such a syndrome disappears after a few days from the onset of menstruation.

Treatment
Two methods are used to treat this disease: therapy or surgery. Therapy involves the use of medications that are focused on normalizing the functional characteristics of the hormonal system and immunity. Surgical treatment, in turn, is based on the restoration of the normal anatomical features inherent in the genital organs. In addition, surgery involves the removal of the maximum number of foci in the area of ββtheir localization. Diagnosis of the disease is carried out by a gynecologist, who should be consulted if any of the above symptoms occurs.
If the conservative treatment is ineffective with signs and symptoms of adenomyosis, surgery should be performed. Operations can be radical, for example, in such cases, a panhisterectomy is performed along with a hysterectomy, supravaginal amputation of the uterus, and so on. Or organ-preserving operations can be carried out, for example, performing endocoagulation of foci of endometriosis.
Indications for endocoagulation are the presence of endometrial hyperplasia, suppuration or adhesions, which prevent the egg from entering the uterus. Another indication in this case is the absence of any effect after treatment with hormonal drugs for three months, along with contraindications to hormonal therapy.
As part of the indications for removal of the uterus, the progression of adenomyosis among patients older than forty years is considered, along with the ineffectiveness of conservative treatment and organ-preserving surgical interventions, a diffuse or nodular form of pathology in combination with myoma, and, in addition, the risk of malignancy.

In the event that adenomyosis is detected in a woman who is planning a pregnancy, she will be advised to make attempts to conceive no earlier than six months after a course of conservative treatment or endocoagulation. During the first trimester, patients are prescribed gestagens. The question of the advisability of hormonal treatment in the second or third trimester with symptoms of adenomyosis in women is decided taking into account the result of blood tests for the content of progesterone in it. Pregnancy is a physiological menopause, which is accompanied by serious changes in the hormonal background and has a positive effect on the course of the disease, which reduces the growth rate and spread of heterotopic endometrial cells.
Adenomyosis is a chronic disease with a high probability of relapse. After conservative treatment, and, in addition, organ-preserving surgery during the first year, relapses of this disease can be detected in every fifth woman of reproductive age. Over the course of five years, the relapse process is observed in more than seventy percent of patients.
In women of premenopausal age, the prognosis for the development of adenomyosis is more favorable, which is due to a gradual decrease in the functioning of the ovaries.
