"Tubocurarine chloride" is a muscle relaxant and has a peripheral effect. The instructions indicate that it is able to block neuromuscular transmission.
Composition and form of release
The main component of this drug is tubocurarine chloride, which is contained in a dosage of 0.01 g. As additional substances, glycerol - 0.3 g and special water for injection are added.
The agent in a concentration of 1% is poured into ampoules of 1.5 ml. Then they are laid out in packages of 25 pieces. Also, the drug is placed in 10 ml bottles, which are packaged in boxes of 5 pieces.
pharmachologic effect
“Tubocurarine chloride” is a non-polarizing muscle relaxant that can block neuromuscular transmission. So, it acts on the skeletal muscle N-cholinergic receptors. The relaxation effect is manifested gradually and reaches a peak after about 5 minutes.
The relaxation of muscle mass occurs sequentially. It all starts with the fingers of the upper limbs, and then the muscles of the eyes, lower limbs, back and diaphragm follow. The restoration of tone occurs in the reverse order and begins with the muscles of the diaphragm.
The mechanism of action of “tubocurarine chloride” releases histamine from the tissues and can trigger muscle spasms in the bronchi.
Indications for use
This drug is used for muscle relaxation during the connection of the patient to the artificial lung ventilation apparatus (mechanical ventilation). It is also prescribed for pharmacological or electrical induced muscle cramps and during the diagnosis of myasthenia gravis.
In traumatology, it is used for reposition of bone fragments and repositions of dislocations. In psychiatry, for the prevention of schizophrenia and for the treatment of seizures. And in neurology - with epileptic statuses and seizures of a different origin.
Contraindications
The drug should not be used for individual intolerance and hypersensitivity to the components that make up the composition. Also, "tubocurarine chloride" is not prescribed for patients with liver failure in chronic renal failure and the elderly.
Instructions for use
The dosage of the drug depends on the type of anesthesia that is administered to the patient. The instructions for “Tubokurarina chloride” say that it is administered intravenously.
In adults, the drug provokes temporary paralysis of the muscles of the respiratory tract at a dose of 16-26 mg. To achieve long-term exposure, the drug is repeated with a dosage reduction of 2 or 1.5 times. For early termination of the drug is administered "Proserin."
Side effects
In patients after the use of the drug, a rash, swelling, collapse, bronchospasm, tachycardia and a decrease in pressure (arterial) can occur. Bradycardia, hypersalivation and allergic manifestations due to histamine liberalization may also develop. Less common is cardiac arrest or rhythm disturbance.
Overdose
With an overdose of the drug, prolonged muscle relaxation can be observed. Treatment in this case is carried out with the help of patency of the respiratory tract and the introduction of neostigmine or pyridostigmine into the body.
The described actions should be taken at the first symptoms of an overdose.
Interaction with other drugs
The effectiveness of "tubocurarine chloride" weaken some substances. These include:
- acetylcholine;
- potassium;
- anticholinesterase agents.
Low temperatures and hypothermia can also reduce exposure.
Anesthetics (inhalation), calcium salts, quinine, some antibiotics (streptomycin, kanamycin, neomycin) and other muscle relaxants can enhance the effect. Strengthening also occurs with hypokalemia.
Pharmacological group and special instructions
"Tubocurarine chloride" refers to drugs that act on the peripheral nervous system. It acts mainly in the zone of respiratory nerves and pathways. It is an antidepolarizing muscle relaxant.
Only a doctor who has experience in using muscle relaxants in preparation for oxygen therapy and with intubation of the respiratory tract can use the tool.
