The use of Ketanov tablets or the administration of an intramuscular solution promotes the formation of a racemic mixture of [-] S- and [+] R-enantiomers. By the way, the analgesic effect of the drug is due only to the [-] S-form.
It should be especially noted that this medication does not depress respiration, does not affect opioid receptors, does not have anxiolytic and sedative effects and does not cause dependence.
What should I know before taking this drug or its analogue? βKetanovβ (pills and injections) are only comparable in strength to their analgesic effect with morphine. According to experts, such a tool is significantly superior to all other non-steroidal drugs.
After ingestion or intramuscular administration, the analgesic effect is noted after an hour and half an hour, respectively. The maximum effect of the drug is achieved after 60-120 minutes.
Pharmacokinetic properties
The original medication, like any analogue of Ketanov, is absorbed quite well in the gastric tract after ingestion. The highest concentration of the drug in the blood is achieved approximately forty minutes after taking the pill on an empty stomach (at a dose of 10 mg). It should be noted that the use of fatty and heavy foods can significantly reduce the maximum concentration of the drug in the blood and delay its achievement by sixty minutes.
About 99 percent of the drug binds to plasma proteins. With hypoalbuminemia, the number of free substances in the blood increases. As for the bioavailability of the drug, it is approximately 80-100%.
When administered intramuscularly, the drug "Ketanov" is rapidly and completely absorbed into the bloodstream. Analogs (Ukraine produces only Ketorolac) are also well absorbed.
After intramuscular injection of 30 mg of the drug, its highest concentration is 1.75β3.2 ΞΌg / ml, and when using 60 mg it is 3.25β5.75 ΞΌg / ml. In this case, the maximum concentration of the drug in the blood is observed after 15-20 and 35-65 minutes, respectively.
Does the medicine pass into breast milk? Both the expensive and cheap analog of Ketanov, as well as the drug in question, can easily penetrate into breast milk. This happens immediately after the first dose of the drug in the amount of 10 milligrams. The highest concentration of the drug in milk at this dose is reached after approximately 120 minutes and is 7.3 ng / L.
About 50% of the administered dose of the drug is metabolized in the liver. As a result of this, inactive metabolites are formed. The main ones are p-hydroxyketorolac and glucuronides. As a rule, they are excreted by the kidneys (about 91%) and through the intestines (about 6%).
The half-life of the drug in patients with normal kidney function is about 5.4 hours. This value increases markedly in the elderly and decreases in the young.

Contraindications

- pain relief before and during surgery (due to the increased risk of bleeding).
We can not say that this medication is prescribed with caution to patients with cholecystitis, bronchial asthma, chronic heart failure, impaired renal function (with plasma creatinine less than 50 mg / l), arterial hypertension, active hepatitis, cholestasis, sepsis, systemic red lupus, as well as in old age (over 65 years) and people with polyps in the mucous membrane of the nasopharynx and nose.

The daily dosage of the drug with intramuscular administration for adults should not exceed 90 mg. For elderly patients and those with impaired renal function, this amount must be reduced to 60 mg. The duration of treatment should not be more than five days.
During the transition from intramuscular administration of the drug to taking the drug inside, the total daily dosage of both forms should not exceed 90 mg for adults and 60 mg for elderly patients or patients with impaired renal function.
Overdose symptoms
When taking high doses of this medication, the patient may experience vomiting, abdominal pain, impaired renal function, metabolic acidosis, nausea, erosive gastritis and peptic ulcer of the stomach.
As a treatment for such conditions, gastric lavage and the introduction of various adsorbents are used. Symptomatic therapy is also performed (that is, they support the vital functions of the body).
Drug interaction
With the combined use of the drug in question with other NSAIDs, the patient may develop additive side effects, and with anticoagulants (including with "Heparin" in small doses and with "Pentoxifylline") - bleeding.
If you take a medication with ACE inhibitors at the same time, then there may be impaired kidney function.
It should also be noted that the use of the drug in question with Probenecid increases the half-life of ketorolac and its plasma concentration.
If you take this medication with lithium, it is possible to lower the renal clearance of lithium and increase its concentration in plasma, and with "Furosemide" - lower its diuretic effect.
During the use of "Ketorolac" there is no need to take opioid analgesics.
